Patient Medical Records and Information Strict guidelines regarding the disclosure, handling and transfer of patient information are observed by all staff. Patients have the right to request access to their own medical records held in both paper and electronic form. Requests should be made in person or in writing to either a GP or the Practice Manager. There is no charge for viewing notes but charges may be made for providing copies, these will be advised at the time of request. Any request for access to a patient's medical records received from a third party such as a solicitor or insurance company must be made with the full consent of the patient. Patient Participation Group In the last two months of 2004, Doctors Kay Bridgeman and Mike Houghton, Practice Partners at Brookside Surgery, responding to a national initiative, invited a number of patients to attend a meeting at the surgery to investigate the possibility of setting up Stretton on Dunsmore’s own patient group. Suggestions, as to who might be interested, came from practice staff, and letters were sent to those identified as potential members. Twenty or so people responded and attended the inaugural meeting. Those attending came from a variety of backgrounds and life experiences and, after much useful discussion, it was decided that a patient group be set up to represent the interests of patients to the practice and act as a sounding board for patient and patient/practice related matters. The membership comes from virtually every village served by our practice and we do have a good cross section of members both by age and sex. Where available the phone number is also shown. Please make contact with them. We want to know how you feel about your local Medical Practice and how it looks after you. PURPOSE We discussed a range topics at our first few meeting but spent some time on developing thoughts about our purpose and reason for our existence out of which, in due course, came our agreed mission statement. A copy of this is also available at the surgery, as are copies of the minutes of our meetings . One thing is abundantly clear. None of our members see the Group as primarily being a fund raising body and have no intention of becoming one. Our job is to act for and with our fellow patients. It is also clear that all members are very genuinely committed to helping the Practice maintain its good reputation with patients. We believe, as patients ourselves, that we get a very fine service from Brookside Surgery and owe the Doctors and Staff much. That does not mean however that we wear blinkers, or rose coloured spectacles, and if you wish to raise any matters with us which are important to you, or can think of any way in which that which is currently available might be improved, please make contact. We want to hear from you. Meetings Dates Meetings are generally held on the first Monday of every other month at 7.30 at the Surgery. All of our meetings are attended by representatives of medical and other practice staff who contribute significantly to our discussions and conclusions. We are on your side.
